How to Read Your Explanation of Benefits

Q. What is the Policy #?
A.

The 2008-2009 Student Injury & Sickness Insurance Policy # for Domestic students is CLA508E.

The 2008 - 2009 Optional Student Injury and Sickness Insurance Policy for Domestic students is CLA608E. 

The 2008 - 2009 Student Injury & Sickness Insurance Policy for Domestic Students is CLA608E. 

The 2008 - 2009 Student Injury & Sickness Insurance Policy for the International Students is CLA708E. 

The 2008 - 2009 Optional Student Injury and Sickness Insurance Policy for International Students is CLA808E.

Q. What is the effective date?
A. August 15, 2008 for students selecting annual coverage. Please see the brochure for other coverage periods (See sample brochure under "Your Coverage")

Q. What is the Policy expiration date?
A. August 19, 2000 for students selecting annual coverage. Please see the brochure for other coverage periods (See sample brochure under "Your Coverage")

Q. If I missed the original registration or waived coverage initially and now want the coverage, how do I go about getting it?
A.
You should send your check or money order for the applicable premium directly to, Bollinger, Inc./Attn.: College Dept., 101 JFK Parkway, Short Hills, NJ 07078. Your coverage will begin on the date we receive your premium.

Q. How do I know if I am eligible to take the Student Injury & Sickness Insurance?
A. All Domestic students taking seven or more credit hours (six or more for Domestic graduate students) are eligible to enroll in the student Injury & Sickness insurance.  All International students taking one or more credit hours are required to enroll in the International Student Sickness and Injury plan unless proof of alternate comparable coverage is shown.

Q. Can I get the insurance  to cover my spouse and/or children?
A. Yes, please refer to the brochure for details. (See sample brochure under "Your Coverage). An enrollment card is available for your convenience.

Q. What does this insurance cover?
A. The Student Injury and Sickness plan covers medical expenses which arise from an accidental Injury or Sickness which occurs while you are covered under this program.

Q. Does the Policy pay all medical bills in full?
A. In most circumstances, it will not. There are exclusions and limitations in the Policy, so you will need to refer to the brochure. Click Here to see a sample Coverage.

Q. May I go to any doctor?
A. Yes you may. However, you are encouraged to go to the student health service first whenever possible and then go to a physician participating in the First Health Preferred Provider Network. Click here for a listing of Preferred Providers

Q. Where do I get a claim form?
A. You can Click Here to print one out or one will be mailed to you upon notice of a claim.

Q. How do I file a claim?
A. Attach your bills to a completed claim form and mail to Bollinger Inc., P.O. Box 727, Short Hills, New Jersey 07078 - 0727. Only one claim form is needed per accident or illness. Subsequent itemized bills may be sent and without completing another claim form. Just indicate that you are a University of Louisiana Lafayette student and the date of your accident or illness.

Q. With this insurance, must I go to the University's Health Center first?
A. You are encouraged to go to the Student Health Center first whenever that is possible. However, it is not mandatory in order to be covered by this insurance plan.

Q. What if I am out-of-state and I get sick or injured?
A.
This insurance plan will cover you for claims which occurred out-of-state. You must follow the same procedures in filing a claim as if it happened here in Louisiana.

Q. How do I get an ID card?
A.
It is attached in the enrollment card. Or, you may obtain one by clicking here.

Q. What is the Policy's maximum benefit?
A.

The maximum benefit for the insurance is $15,000.00
The maximum benefit for the Domestic Basic Sickness and Injury Optional Domestic  Injury and Sickness Insurance is $50,000.
The maximum benefit for the International Basic Sickness and Injury insurance is $25,000.
The maximum benefit for the International Injury and Sickness insurance is $7,500.
