Q. How do I know if I am eligible to take the Student Accident
& Sickness Insurance?
A. All students taking 6 credit hours are eligible to take the Student
Health Insurance Plan.
Q. Can I get the insurance to cover my spouse and/or children?
A. Yes.
Q. What does this insurance cover?
A. Accident & Sickness subject to the plan terms
Q. Does this policy pay all medical bills in full?
A. No, in most cases there are coverage limitations.
Q. May I go to any doctor?
A. Yes , but discounts are available if a
First
Health Provider is used.

Q. How do I file a claim?
A. Attach your bills to a completed claim form and mailed to Bollinger
Inc., P.O. Box 727, short Hills, New Jersey 07078 - 0727. Only one claim form is
needed per accident or illness. Subsequent itemized bills may be sent and without
completing another claim form. Just indicate that you are the Northern Kentucky
University student and the date of your accident or illness.
Q. Does this policy cover me for annual physicals?
A. No.
Q. Is there a deductible on this policy?
A.No.
Q. With this insurance, must I go to the University's Health
Center first?
A. Yes.
Q. What if I am out-of-state and I get sick or injured?
A.Coverage is valid anywhere in the world.
Q. How do I get an ID card?
A.It is attached in the enrollment card. Or, you may obtain one by
clicking here.
Q. What is the policy's maximum benefit?
A. $20,000
