Q: What is the policy number?
A: The 2007-2008 policy number is C607D.
Q: What is the effective date:
A: August 15, 2007 or the date Bollinger Inc. receives your premium check, whichever
is later.
Q: What is the policy expiration date?
A: August 15, 2008. Even if you leave school or graduate, your coverage will remain in
force until that date.
Q: If I missed the original registration or
waived coverage initially and now want the coverage, how do I go about getting it?
A: You can send your check with the annual premium amount of $123.00 directly to
Bollinger Inc./ATT: College Dept. 101 JFK Parkway, Short Hills, NJ 07078. Your
coverage will begin on the date we receive your payment. If you want coverage for the
second semester only, the cost is $74.00.
Q: How do I know if I am eligible to take the
Student Accident & Sickness Insurance?
A: If you are a full time student, part-time student, or graduate student, you are
eligible.
Q: Can I get the insurance to cover my spouse
and/or children?
A: Yes, to do so you must go to the Business Office to pick up a separate application, or
follow this link for a printable application, which needs to be completed and sent in with
the appropriate premium amount
Q: What does this insurance cover?
A: This insurance covers medical expenses which arise from an accidental Injury or a
Sickness which occurs while you are covered under this program.
Q: Does the Policy pay all medical bills in
full?
A: In most circumstances, it will not. There are exclusions and limitations in the policy,
so you will need to refer to the brochure (See sample brochure under
"Brochure").
Q: Can I go to any doctor?
A: Yes you can. However, the Insurance Company does work with the Consumer Health Network
(CHN) which is the largest Preferred Provider Organization network of physicians and
hospitals in New Jersey. Going to a CHN Network Physician or
Hospital would in most cases result in lower charges which would directly lower any
co-payments that you would have to pay. Please contact the Health Center at New Jersey City University
for more details on the network.
Q: Where do I get a claim form?
A: You can pick up a claim form at the Health Center or we will mail you one
upon receiving first notice of your claim or you can print one from this link.
Q: How do I file a claim?
A: Attach your bills to a completed Claim Form and mail to Bollinger Inc., P.O. Box
727, Short Hills, NJ 07078-0727. Only one claim form is needed per accident or illness.
Subsequent itemized bills may be sent in without completing another claim form. Just
indicate that you are a New Jersey City University Student and the date of your accident
or illness.
Q: Does the Policy cover me for my annual
physicals?
A: For all persons age 17 and older, an annual consultation and tests are covered under
the policy.
Q: Is there a deductible on the Policy?
A: No, there is no deductible and this insurance is primary, so you need not go to any
other applicable insurance before collecting under this plan.
Q: With this insurance must I go to the
University Health Center first?
A: You are encouraged to go the University Health Center first whenever that is
possible. However, it is not mandatory in order to be covered by this insurance plan.
Q: What if I am out of state when I get sick
or injured?
This insurance plan will cover you for any illness or Injury which occurs out of state.
You must follow the same procedures in filing a claim as if it had happened here in New
Jersey.
