1. Fill out the Amateur Sports Insurance Underwriting Information questionnaire.

• Complete the General Information section, providing detailed answers to all questions. If a particular question is not applicable to your operation, write "n/a" or "none."
• Detail all relevant information on your current and prior insurance programs, including carrier’s name, premiums paid and losses for the past 3-5 years. Please provide hard copy loss runs from your current/prior insurance carriers. If no loss runs are available, please provide us with a letter stating why runs are not available, and outline any losses or claims that your organization has had in the past 3 - 5 years. List losses by policy type (i.e. all Accident claims, and all Liability claims). If you have had no losses during this period, please indicate that in your letter.
•  For each sport that you sponsor, you will need to complete the individual section of the questionnaire on that particular sport.
•  If you use batting cages, sponsor camps or clinics, or own (or lease on a long term basis) the premises/facility, please complete the individual sections related to those areas as well.
• Be sure to provide details on fund-raising, travel or other miscellaneous activities that you sponsor, in order for the Bollinger Insurance underwriters to properly quote on your association.
• Please include a copy of the waiver form that you currently use, and a copy of your organization’s written Emergency Medical Management Plan or your Emergency Crisis Management Plan.
• If your organization has written guidelines on protecting players from, preventing, recognizing signs of and reporting Sexual Abuse and Molestation, please include these guidelines with your underwriting information.

2. Fax or send your underwriting information and supporting materials to Bollinger's Sports underwriters. Our Sports Fax line is 973-467-0759. We will review your submission and follow up with you on any questions we may have.

3. A proposal will be sent to you usually within 1 - 3 business days after we receive your complete information. If you are under a deadline for the proposal, please inform our underwriters. We can rush a quote within 24 hours, if need be.

4. If you would like to explore other options once you receive your proposal from Bollinger, please let us know. Our underwriters will be able to quote different limits, higher or lower deductibles, additional coverages or payment plans to give you the package of insurance coverages that you need. Just give us a call for any revisions that you would like to see.

5. To bind coverage under the Amateur Sports Program, we will need premium payment in full for accounts with premiums under $5,000. For accounts over $5,000, different payment plans are available, and we will need payment of the first installment to bind coverage.

6. A binder of insurance will be issued upon receipt of your payment. Plus, if you need any certificates of insurance as proof of coverage for facility or field owners, we will issue those for you as well at that time.
