Dear Students, Parents and Guardians:

Flagler College is pleased to make available an Injury & Sickness insurance plan to our students. The details of all coverage and instructions for enrollment are available on the special web site for our plan: www.bollingerinsurance.com/flagler. To see a copy of the Schedule of Benefits for next year, please click here. All of the other terms, conditions, limits and exclusions will be the same as “Plan II” had for the 06/07 school year, which can still be viewed on the current plan web site under the “Coverage” menu on the web site.

Please take a few moments to review and take action on the following information.

A. All registered students taking 6 or more credit hours are Eligible for enrollment in the basic accident and sickness plan. There were several improvements made to this plan over the last year. Several benefit limits were increased, including an increase to the prescription drug benefit up to $500 per policy year after a $20 deductible per prescription. This plan now has a deductible and co-pay, but it offers higher benefits and a higher medical maximum per injury/illness. The co-pay is 20% when you see a health care provider belonging to the PPO network, First Health (formerly First Health). If you choose to see a provider that is not in the First Health network, the co-pay is 40%. Please consider what is best for your circumstances and do not hesitate to call Bollinger Insurance at (866) 267-0092 to ask questions.

For the Basic plan, this is the fee charged to your student account:

Students entering College the Fall 2007 semester: $350 (This is for the entire year)
(New students beginning in Spring 2008 or lost their prior coverage since Fall: $220)

To be exempt from this coverage and fee, the student must sign the waiver on the payment worksheet showing evidence of comparable coverage no later than August 1 for the Fall Semester or December 1 for the Spring Semester.

B. Students who would like to consider an enhanced plan may select an optional major medical, to increase the aggregate maximum per Injury or Sickness to $50,000. The optional Major Medical Benefit begins payment: after the Basic Maximum Benefit of $25,000 has been paid for or becomes payable by the Company. The Company will pay 80% of additional incurred Covered Medical Expenses for in network treatment. The Company will pay 60% of additional incurred Covered Medical Expenses for out of network treatment. Payment will not exceed the Major Medical Maximum Benefit of $25,000 for Injury and $25,000 for Sickness, for a total aggregate benefit limit of $50,000 per Injury or Sickness.

Only if you opt for the Major Medical too, this is the fee charged to your student account:

Students entering College the Fall 2007 semester: $475 (This is for the entire year)
(New students beginning in Spring 2008 or lost their prior coverage since Fall: $345)

Forms to be completed and returned:

1. If you have comparable coverage and want to be exempt from this student insurance plan, fill out the insurance waiver section on your payment worksheet for tuition and fees.

2. If you want to elect for the optional major medical buy-up, instead of the Basic Plan, select Upgrade to Catastrophic on your payment worksheet and return to the College by August 1, 2007 for the Fall semester (December 1 is the date for new students starting at Flagler in the Spring semester or returning students who no longer have other insurance and must take this plan starting in the Spring).

Review the enclosed brochure and compare. In addition you can review the prior school year brochures for “PLAN I and PLAN II” on the web site. Note: Disregard “Plan I” as “Plan I” will no longer be offered. “Plan II” is now going to be the basis for the Basic plan, except it will have a $25,000 maximum in 2007-2008 (plus the option to purchase the major medical for an additional $25,000 after the Basic medical maximum is reached).

Also make sure that you are still eligible for any coverage you are keeping. Some coverage’s end when you attain a certain age. We hope this plan provides you the opportunity to secure quality health insurance at a reasonable cost.

If your personal insurance is an HMO be aware that it may not pay for treatment outside their Network area when it is an emergency and will not pay for treatment from doctors out of their area without prior written permission. We suggest you check with them to see how they handle out of area claims.

Finally, before you make your decision, ask questions. Just call our plan administrator Bollinger, Inc., toll free at 1-866-267-0092. They will be glad to help you with any questions you may have. Also know that this plan works best as a supplement to any existing coverage you have, therefore it is recommended that in most cases you should retain your present coverage.
