A: August 15, 2007 for student selecting annual coverage.

Q: What is the policy expiration date?
A: August 14, 2008.

Q: How do I know if I am eligible to take the Student Injury & Sickness Insurance?
A: All students attending Flagler College taking 6 or more credits are required to purchase this insurance plan unless proof of comparable coverage is furnished.

Q: Can I get the insurance to cover my spouse and/or children?
A: No.

Q: What does this insurance cover?
A: This insurance covers medical expenses which arise from an accidental Injury or a Sickness which occurs while you are covered under this program.

Q: Does this policy pay all medical bills in full?
A: In most circumstances it will not. There are coverage limitations and exclusions in the policy, so you will need to refer to the brochure. This is an excess insurance plan, which means it pays secondary to any other insurance coverage you have. For Plan I, the first $100 is covered as primary but after that your other insurance available at the time must be used first.

Q: Can I go to any doctor?
A: In a non-emergency situation, you are strongly encouraged go to the Health Center first, if possible. Arrangements have been made with physicians who participate in the First Health Preferred Provider Network to provide discounted services to Flagler University insured students. Use the link provided on this site  to search for a provider in the network.

Q: Where do I get a Claim Form?
A: You can print one from this web site (See "Claim Form").

Q: How do I file a claim?
A: Attach your bills to a completed Claim Form and mail to Bollinger, Inc., P.O. Box 727, Short Hills, NJ 07078-0727. Only one Claim Form is needed per accident or illness.

Q. What if I am out of state when I get sick or injured?
A. This insurance plan will cover you for any covered Sickness or Injury which occurs out of state. You must follow the same procedures in filing a claim as if it happened in Florida.

Q: How do I get an I.D. Card?
A: An I.D. Card may be printed from this site. Please be sure to fill out that you are at Flagler College and the correct policy number for your plan. The policy # is CFL405D.

Q: What is the policy's maximum benefit?
A: $25,000 for each Injury or Sickness. (Increased to $50,000 if major medical is elected.)
