A. Attach your bills to a completed Claim Form and mail to Bollinger Inc., P.O. Box
727, Short Hills, NJ 07078-0727. Only one claim form is needed per Injury or illness.
Subsequent itemized bills may be sent in without completing another claim form. Just
indicate that you are a Bryan College Student and the date of your Injury or
illness.
