Q. How do I file a claim?
A. Attach your bills to a completed claim form and mailed to Bollinger Inc., P.O. Box 727, short Hills, New Jersey 07078 - 0727. Only one claim form is needed per accident or illness. Subsequent itemized bills may be sent and without completing another claim form. Just indicate that you are the Northern Kentucky University student and the date of your accident or illness.
